The admission process at Three Rivers Montana involves two steps. The Admission Packet needs to be submitted and reviewed by admissions counselors to determine the appropriateness of the child for the program. Once the child is accepted you will be asked to fill out and submit the Enrollment Packet before the child arrives at Three Rivers Montana.

Our admission and enrollment packets are available in two formats from this page.
The Adobe PDF format allows you to download and print the packets from your home computer and printer. Fill in the forms then fax or mail the packets back to Three Rivers Montana. Fax number and address are listed above.
To download the PDF forms click on both links below and save the files to your desktop. When the downloads are complete double click on the files to open them, then choose FILE -> PRINT. You may close the document once it has printed.

The Application and Enrollment Packets may also be filled out from your computer desktop. From this page download the PC or Macintosh files to your computer by clicking on the appropriate link below and saving the file to your desktop.

For PC users click on link D2 below to begin downloading the installer. When prompted by the browser, save the installer to your computer desktop.

After the installer download is complete:

1. double click on TRMApp-EnrollWin.exe. This will create the "TRMApp-EnrollWindows" folder on your desktop.
2. The folder contains four files:
   1. ReadMePC.pdf with instructions
   2. FillerSetUp.exe for installing the application
   3. and the Application and Enrollment documents for Three Rivers Montana.
3. Double click on ReadMePC.pdf for detailed instructions
   or double click on FillerSetUp.exe to install

After filling out the Enrollment Packet if you want to fill in multiple copies of the Confidential Information Release form you may click here to download a pdf version you can print, fill in and fax back to
406-388-5275. If you prefer to fill the form in at your computer call Three Rivers Montana 877-221-1115. We will email you an electronic version to be filled out with the same software used for the Application and Enrollment Packets. Once completed the forms may be emailed back to Three Rivers Montana.
