This packet includes an Application Form, Medical Release Forms, Confidential Medical and Social History Forms, and Interstate Compact Form. If you have questions about any portion of this Admissions Packet, please call and ask our Admissions Department for additional guidance - (877) 221-1115.

In applying for admission to THREE RIVERS MONTANA, please carefully review the following steps:

• Send the completed and signed admission forms and information to the address below or fax them to 406-388-5275. You do not need to have your son or daughter sign the forms. THREE RIVERS staff will explain the forms to the student on admissions day and will ask for the student's signature then.
• Call the Admissions Office and schedule an admission date.
• You may complete the enrollment packet after your child has been accepted to Three Rivers. The enrollment packet must be faxed or mailed to Three Rivers before the actual admission date.
• If possible, please include a recent photograph of your child with the admission forms.
• Non-Montana residents should complete and sign the Interstate Compact Form according to the enclosed instructions.  Include this form when submitting the rest of the enrollment packet.
• Please ask our admissions director about financial arrangements.

• Review your application and contact you with any further questions or requests.
• Verify your arrival time.
